Attached is the necessary paperwork for your athlete (Grades 6-12) to bring. He/she must have page 2 and 3 signed by the parent/guardian in the appropriate spots prior to having a physical. The parent/guardian does not have to accompany the athlete to the gym if the paperwork is signed.
